Nothing on this page is saved or sent anywhere.
It lives in this tab only — no autosave, no draft, nothing reaches the Hub. Print it, get it signed,
and file it with the child’s care plan. Closing or reloading the page loses what you have written.
Until the Google Workspace move is finished we cannot hold any more children’s information in the Hub,
so this review is a paper document for now. That is deliberate, not an oversight.
🌱 Care plan review
The care plan was written before your child’s key person had met them. This is where we check it against
the child we actually see — and say what changes. Fill it in with what you have noticed, go through it with
the parent, and both sign it.
When a care plan is reviewed
Owner’s decision, 14 October 2026. Not yet written into the Settling In policy — see the note at the bottom.
2 weeks A settle check, not a full review. Key person only, ten minutes. Does the plan match the child we are seeing? Most of the drift shows up here, in sleep and food.
6 weeks The first full review. Long enough that the child is genuinely settled, short enough that the parent still remembers what they wrote.
Every room move The one most likely to be missed. A new key person inherits a plan written for a different room, a different nap pattern and different staff.
3 × a year Then on a cycle, three times a year. Where a child also has a SEND support plan, hold the two together so parents are not pulled in twice.
Any time A new allergy or medication, a stay in hospital, a pattern in sleep or behaviour, or a parent asking.
The progress check at age two is a separate document and stays separate (owner, 14 Oct 2026).
Hold both in the same conversation with the parent if the timing lands together, but file them apart.
See the two-year check course.
📄 The plan we were given
The care plan written from the parents’ fact file, before this child started.
Load it and it fills the Before column — what we were told — so you are not
typing it again from memory. It only fills boxes that are still empty.

This review
Before · Now · Next eight areas
Before is what the parent told us at the start — copy it from the original care plan and then leave it
alone, every time. It is the record of what we were told, and keeping it frozen is what makes the change visible.
Now is what the key person actually sees. Next is what changes, who does it, and by when.
If nothing has changed in an area, write “no change” — that is a finding too.
Checked before this is filed
The allergy, medication and SEND lines are the ones that must not drift. A second person checks them against the current records, not against the old plan.
Where the child has a SEND support plan, our SEND policy says it is reviewed at least termly — three
times a year here. Where they have an Individual Health Care Plan, that is reviewed by the health team
that issued it, not by us (Medication policy v1.0). This review does not replace either; it sits alongside them.
Signed
The parent signs every version, not only the ones where something medical changed (owner, 14 Oct 2026). The previous version is kept, never overwritten — see our Access, Storage and Retention of Records policy.

📧 Send it to the parent to read and sign
The same way the safeguarding acknowledgement does it: the review is saved as a numbered
version, the parent gets a link, and their signature is stored against that version. Live since
25 October 2026.
What happens when you press it: the review is saved as a numbered version, the parent is emailed a link to
care-plan-sign.html?t=… showing the Before, Now and Next in full with a box to sign in, and the
signature is stored against that version. The email itself carries nothing but the child’s first name and the
link — no sleep, food, allergy, medication or SEND detail ever goes in an email body.
🔒 What happens to a care plan once it is sent.
Sending a parent a link means storing that care plan — sleep, food, allergies, medication, SEND —
plus their signature and email address. That is why this was switched off from 15 October until
25 October 2026: the Hub’s sheets sat on a personal Gmail with no data processing terms with
Google. They are now owned by the organisation on Google Workspace, with those terms accepted, so the
storage is on a footing we can stand behind (OUTSTANDING.md,
the DPIA).
The email carries nothing but the child’s first name and the link. No sleep, food, allergy,
medication or SEND detail ever goes in an email body — it is all behind the link, which is a one-time
token for that parent and that version.
You can still print instead. If a family would rather have paper, print the review or save it as a
PDF and take the signature by hand. Same conversation, same document, nothing stored.
📘 This cycle is policy now.Settling In v1.1
(October 2026) sets out the review rhythm below: two weeks, six weeks, every room move, three times a
year, plus triggers. It also says the progress check at age two stays a separate document and that
parents read and sign every version. Until 25 Oct 2026 this page carried the cycle as the owner’s
decision of 14 October, recorded with its date but not yet policy.
