🩹 Report an accident or incident

Someone hurt, something that happened, or something that nearly did — a child, a member of staff, a visitor. If you saw it or found them, it’s yours to record (Accidents and First Aid policy). Give first aid first.

It goes to your manager, who reviews it, makes sure parents have signed and decides if it needs reporting further.

🛡️ Not this form if you think a child may have been hurt on purpose, or the injury doesn’t match what you were told. Tell the DSL now, then use Safeguarding: cause for concern.

What was it?

Who was hurt?

When and where

What happened

The facts: what they were doing, what happened, what you saw. Not what you think caused it.

Head injury: watch them closely, tell the manager now, and parents should be told straight away and given head-injury advice.

The parent

The parent is shown the record and signs it the same day, whether or not their child was hurt. Your manager will check it’s been signed.
