⚑ SEND: something noticed about a child
You have noticed something and you want somebody to know. That is the whole purpose of this form. It goes to the SENCo and your manager, and it is written down with today’s date on it.
Raising something is not the same as deciding what it means. Nobody is being referred, labelled or diagnosed by you filling this in. The next step is a conversation.
Which child
First name and the first letter of their surname is enough. We do not need their full record here.
What you have noticed
Write what you have actually seen or heard, not what you think it means. The seeing is your job and it is the part nobody else can do.
The family
You do not have to have spoken to them. If you have not, say so — that is often the right answer and the SENCo will decide who speaks to them and when.
If a child is at risk of harm, this is the wrong form. That is a safeguarding concern and goes straight to the Designated Safeguarding Lead, now, in person.
