π Medication
Consent, each dose given, and the parent’s signature at hand-over and collection — the paper form, kept here instead.
What our Medication policy v1.0 requires, and this page enforces.
Written permission for each and every medicine, for that brand at that dose — if either changes,
it needs a new record. A senior member of staff gives it and a second member of staff checks the
child, the medicine and the dose. The parent is asked when it was last given before nursery, and
signs twice: at hand-over and at collection. A refusal is recorded.
This page records what was consented to and what was done. It never decides whether something may be given.

New medication record
One record per medicine, per dose. A change of brand or dose needs a new one.
Standing consent is the one filled in at registration for fever relief or antihistamine.
Required for prescribed medicine.
Checked against the instructions on the medicine. This is the only dose that will be given.
A dose above the label is only given with written instructions from a health professional.
These are not given without training specific to this child.
The parent signs here to give permission. Nothing is given without it.
